No, Muriel. I am saying that "None of this structure is being used". It can also be said "All this structure is not being used" (While you said "not all of this structure", which imples that only part of it is used, which is not what the author tried to say.

Quisiera expresar mi "sympathy" por Muriel, porque la frase está fatal en español es ambigua. No obstante, yo tengo varios puntos claros:

1. Se refiere a las estructuras que ha mencionado en el párrafo anterior, es decir, las estructuras de las farmacias del Pueblo.

2. Se refiere a TODAS las estructuras, porque para referirse a una parte (y no al "todo") de las estructuras, HUBIERA INVERTIDO EL ORDEN de la negación: "NO todas...."

3. El todo de una negación equivale a la negación de un todo, es decir, lo correcto es español hubiera sido decir "NINGUNA DE ESTAS ESTRUCTURAS..."

4. Hay una posible razón psicológica para no usar NINGUNA, y es que se trata de una negación coyuntural, temporal, provisional, debida a "la respuesta nacional al VIH".

5. La palabra "TODO" no hay que tomarla literalmente: expresa el contraste entre el esfuerzo que han realizado las farmacias dle pueblo y la respuesta (negativa) nacional. Es decir, al decir "TODA esta estructura" no se refiere literalmente a las estructuras, sino que es una "deixis de situación" se refiere a que todo el esfuerzo ha sido (al menos por ahora) en vano.

La traducción no tiene por qué ser literal, basta con que exprese el contraste (con un HOWEVER, por ejemplo):
