This appears in an A&E report from Spain, in Physical Examination section:
Sol. continuidad en "M" de 2 cm ... sin perdida de sustancia noble. I have no idea what this means! Help!

Just a guess: Could "noble" be "notable"?

After all, doctors tend to qualify their statements, and no/never/all/always are practically(!) non-existent in medicine. (Very helpful in multiple choice exams at med school way back when: all options containing "always", "never", "no" or "all" could be excluded without reading any further ;-)

I can't find much for "noble", maybe just substance/sustancia is sufficient.
