The Lepine medical dictionary gives 'declivous, inclined, sloping'.

However, the only examples of 'declivous' I find on the Ouèbbhe in relation to the lungs (and there are quite a few) are in English-language articles written by Italians. ...

I strongly suspect it is simply the 'lower part' of the lung (irrespective of whether the patiient is in the Trendelenburg position or not).
