I'm not sure that ciudadanía would be the correct term in this context....It's in a form that a student and his/her parent's must sign and return to the teacher.

Verbatim: Please sign this notice so that my teacher knows that you are aware of this problems and that you know my academic or CITIZENSHIP grades will drop significantly if this continues.

conducta, comportamiento

This will mean that his "notas, calificaciones en conducta bajarán si ese comportamiento continua" This is the term schools in the U.S. use for conduct, behavior, some schools give monthly "awards for citizenship" sometimes they give a decal that says something like "mi child is a citizenship award recipient at (name of the school)

cívica

Hi Hardy,

In Bolivia, we called this "cívica," which refers to civic acts, patriotic festivals, and overall patriotic behavior.

civismo ( o conducta)

I think it is refered to the student's behaviour (Citizenship grades = notas en conducta o civismo)
