remise en main propre pour faire valoir ce que de droit
"Certificat etablie a la demande de l'interesse et remis en main propre pour faire valoir ce que de droit."
This phrase is on a certificate from a doctor's surgery to confirm that a person who is requesting the adoption of a child is in good health. This is the final sentence of the certificate and I was wondering if there was an equivalent set legal phrase - or perhaps just some help with a nice neat translation!!
