Hi all -
I have a document I am translating coming from a hospital however it has an ordinary business heading at the top of the form.It lists three phone numbers as such:
Tel. +039.000.000.000
Fax.+039.000.000.000
CUP +039.000.000.000
I have no idea what the CUP stands for. (It\'s probably something obvious but I am stumped.)
Any help would be greatly appreciated!

I am pretty sure this is what it stands for:
"COS’E’ IL CENTRO UNICO DI PRENOTAZIONE REGIONALE (CUP)?
E' il sistema di accesso alla prenotazione di visite ed esami nelle strutture sanitarie del territorio regionale. Here it's simply called "Call Center":

Il CUP ha un call center sanitario che nasce con l'obbiettivo di stabilire un rapporto più semplice e diretto fra i cittadini e la sanità."

However, this seems too general a term; the reference to "prenotazioni" indicates that it is primarily the number you call to book an appointment.
In practice the equivalent might well appear simply as "Appointments". But perhaps "Central Appointments" might convey the idea of channelling all the calls through one call centre, a "centro unico" to coordinate the operation.

Curiously, many of the US hospitals that have a "central appointments" number seem to be military. It's pretty common in NHS hospitals in the UK. Maybe there's a different term more widely used in the US. Anyway, that's what it refers to.
