German to English Bus/Financial bank statement
This text deals with the identification of bank customers. So far I've been translating this as registration form and am not happy with this solution. They are capturing data on this form. Every time you withdraw or deposit money you get what I'd call a receipt.

Da in diesen Fällen die Ausweiskopie entfällt, ist der Sachverhalt auf dem Erfassungsvordruck mittels Vermerk „Kunde ist mir persönlich bekannt und bereits unter Konto-Nr. ... identifiziert“ (oder dergleichen) zu dokumentieren und vom identifizierenden Mitarbeiter zu unterschreiben.

I know you have Hamblock/Wessels. Look at the various possibilities for \"erfassen\" and \"Erfassung.\"
Could be: data compiliation form or data acquisition form.

It's the lawyer in me that makes me come up with these esoteric terms.
agree gangels (X) : "Recordation of Title" is a standing phrase in real estate contracts. Somewhat ancient and perhaps not the best in context.

data acquisition form

That is what it is usually called in computerized data acquisition.

Sometimes, when pre-printed forms are filled in by hand, this is also called a "request form".
