i think a generally understandable word would be preferrable here since it's not a legal text, just a little note.

Jumping right into ... ... hair splitting territory here, Sebastian, I'd say the C&D letter is both the (lawyer's) request (i.e. letter) and the declaration to act as requested (if/when signed). -- But I'm not even sure this is relevant, given the rather general context, i.e. Abmahnung as a part of today's daily Internet-Kultur (sort of).

A cease and desist letter (Unterlassungs- und Verpflichtungserklärung) often accompanies an Abmahnung. The Abmahnung is the letter itself, and is called a warning letter. You basically send it to request the recipient to refrain from infringing your rights, etc.
