: I think that this term describes the file that is kept in the hospital during inpatient treatment (for example it includes the vital function tracings,all administered meds etc). However, the whole documentation kept by the family doctor is a medical file

Thanks for your opinion. In the States, in the New England in particular, (patient's) chart is used in reference to medical records. This is very common around here and I should know as I work in medical setting on daily basis. Thanks.
