Title of a legal document in Spanish
I am not sure how to translate the title of this document. The only word in the title is Constancia. It is a document issued by the government that proves that someone has a very specific type of driving licence. This document is from Argentina and the translation is for a company in Canada.
I checked the glossaries first, I found that for other type of documents, they use: Document of Account/Document of Certificate/Document of Record/Writen declaration.
I am asking because I am not sure if any of those make sense for this particular type of document. I will write the first and last paragraph to give more context,

First paragraph: Por medio de la presente se deja constancia de que esta Municipalidad ha otorgado a la Sra. XXXX una licencia de conducir cuyas clases y vigencias se describen a continuación.
Last paragraph: Se extiende la presente a solicitud de la parte interesada para los fines que le convengan enXXX deXXX del XXX
