not an elegant solution but accurate---patient was admitted on DATE and either Discharged on DATE or Not yet discharged as of DATE..

Still having the (symptoms)

I might think that it is about the symptoms the patient has : when entering the hospital these are recorded and some may persist even when leaving . Recording these may allow the family- or other specialist doctor to start an ambulatory treatment when this document of discharge is handed over to them
