Informed Consent Form
"Zeuge (falls zutreffend) in Druckbuchstaben - Unterschrift - Datum"
From a form concerning involvement in a clinical trial. Usually I don't have that much trouble with the bits right at the end, but I'm baffled about "aufklärender Arzt". The meaning of "aufklärend" in this context is not at all clear, and the dictionaries aren't really helping.
I have seen the other question about this - could it maybe be "physician obtaining informed consent" here? TIA for your help.

Physician obtaining informed consent
"Physician obtaining informed consent" is a frequently used expression. The meaning of "aufklärend" is that the doctor informs the patient about the procedure and its risks, benefits and alternative choices and then has the patient sign the "informed consent". See first link below for explanation of how to obtain informed consent and second link with a sample "informed consent" form of a cord blood study.
