: Form is the end result of applying a template and re-model it to your own needs, CORRECT. That's why when you fill up a data-base "maschera", you're filling up the form, not the template (because you're entering data, you're not drafting the template)

I re-read Alexander's text, and I do not think he's talking about filling up forms or entering data, but assembling fields according to predefined sets. Maschera is sometimes used as "form" in Italian, but in this case I think it means template.
