maschera is often used in Italian for "template" in your context.
Another possible term is "preset".

Usually, \"form\" is the end result of applying a template to data stored in a database. The template contains the information about what \"bits\" are to be assembled to produce the final form.

I re-read Alexander's text, and I do not think he's talking about filling up forms or entering data, but assembling fields according to predefined sets. Maschera is sometimes used as "form" in Italian, but in this case I think it means template.
